This form is for Screenwriters wishing to post treatments for sale.

Instructions:

• Complete and print out this form.
• Enclose typed copy of your treatment(s) (up to 15 pages each).
  Treatments may also be submitted on disk or by e-mail. E-mailed treatments will be posted on our web site as soon as check is received and they avoid typing errors.
• Enclose $49.00 check or money order for first treatment and $39.00 for each additional treatment. (Service starts when your Treatment(s) are posted on this site and ends 1 year from that date.)
